Many serious accidents have occurred due to a machine being started when it should have been completely shut down to allow for maintenance or repair. These accidents are usually the result of energy that was not released correctly. Lock-out tag-out (LOTO) systems can prevent these accidents by making sure that the machines remain off until the authorized individual releases them.
The LOTO system consists of a series of locks tags, locks and other devices that are used to isolate, secure or shut off the machine from an electrical source. All "affected" and "authorized" personnel should be trained on the use of these devices. These devices include, but are not restricted to chains, locks, and padlocks. Each device is uniquely identified and labelled, including the date and time at which it was put in use along with the name and title of the person who was authorized to use it and the reason for the lockout or tagout (repairs maintenance, repairs or repairs, etc.). It is imperative to state clearly that these devices are not to be removed until the LOTO procedure is complete.
